Failure rate



Greeting,

 

I would like to receive your comments on the following questions. What is a typical range for fiber failure rate? Is it safe to assume that it is around 500 FITS per mile (or 5e-7 failures/mile/hour)? Secondly, is it just the fiber cut the main source for fiber failure? If you know any document/paper/page that discusses the above you would kindly let me know.
