Angling towards -04

I'm going to try to get an updated draft out "soon" (next week).  I've
made a pass over all tracker comments & think I've addressed most of
them adequately.  Current version is @

If I've missed something that's important please let me know.

One change that has not been made is adding the disclaimer for RFC2119
keywords and/or lowercasing the keywords.   At this point, I see it
as two sides of the same coin & am leaning toward keeping the MUSTs
with the disclaimer about it being *my* opinion, not a binding
standard.
