Duplicate PEN from IANA

  OK... this would seem to be another issue that the guidelines document
needs to resolve.

  If allocating multiple Enterprise IDs for one vendor is not something
IANA will do, the document needs to be updated to specifically request
this.  The current IANA section doesn't specify this.

  If we don't want to allocate multiple Enterprise IDs for one vendor,
then the Extended Attributes document becomes more time-critical.

  Any suggestions?
