social registration

Looking at the registration form for the social, I noted (a) there are 
only two choices for salution, "Mr." and "Mrs.", which is sure to 
exclude, say, those who prefer "Ms." or "Miss" (personally, I prefer a 
blank salutation for myself), and (b) the list of countries is rather 
short and is missing (among others) Japan and Canada.
