It appears that the largest hurdle is that CDN's are not built on a common
basis.  Some CDNs use proximity determination via ping delay to a client's
DNS server, some determine delay to the actual client, some use BGP...  Then
the handling of the variety of URL encoding schemes and support for
various media types becomes quite complex.  However, of the three, I see
Request Routing being the simplest to handle compared to Content
Distribution and Accounting.
