Not to extend the discussion, but this is the point I was trying to make
(clearly less eloquently):

The question is why does the destination care? Answer: because the current
business models are built around optimizing inbound costs for the
destination. If the business model were traffic origination based rather
than termination based, the destination would not care where the originator
inserted the bits. While changing business models is not a function of
protocol design, existence or lack of appropriate tools may have some
influence.
