VLAN-03 strawman

As we work to put the remaining issue(s) to bed on the VLAN/priority draft, 
I thought it would be helpful to have a strawman document reflecting where I 
think we are at the moment:

The current version reflects Paul's most recent editorial issues posting, as 
well as a cleanup of the reference section (several references were not 
actually referenced in the text).   Note also the addition of text 
discouraging use of the attributes in Accounting-Requests (from Hannes' 
issue).
