Tracker: IESG Evaluation state vs. On Agenda flag



Ever since we introduced the On Agenda flag as an orthogonal item in
the tracker, it's been possible to have conflicts between the state
and the flag.  I finally got around to collecting the data required
to find these conflicts, and found 25 documents that are:
- in IESG Evaluation state but not on agenda
  (they should be in a substate e.g. Point Raised, or another state)
or
- in a substate of IESG Evaluation but are on the agenda
  (they should presumably not be in a substate).

I'm worried that the first of these leads to black holes -- a document
is in a state that an AD doesn't have to worry about, but not in a
state where it will move forward without someone's action.

Because of the overhead involved, I didn't fetch the "on agenda" flag
for every single document to check if there were other documents that
were on the agenda but not in IESG Evaluation state (this is sometimes
OK, which is why we introduced the "on agenda" flag -- e.g. something
that's still in IETF Last Call but the LC will end before or on the
telechat date), plus we'll probably notice these because they're
on the agenda.
