Chagnes to logging requirements, markup method for changes

I've updated the logging requirements that deal with 
IP addrs and DNS names in the logs.   See 2.5.6 and 2.5.7
of http://www.port111.com/opsec/latest/

Also, check out the notation on the section headers
indicating the change, the date of the change, and
the source.  Lacking changebars, I think this will
serve so show what changes have been made.
