heath-ppp is listed in TO state:

This doesn't seem quite accurate. This document was discussed in the
pppext WG back in January (with the authors participating). They
agreed to make some revisions, but have not done so yet. When I pinged
them last (in June) they indicated they had been busy and would get a
revision out in a "couple of months".
